How do you complete the Patriot form when it is a POA?

Seriously, it depends upon the wording. Most of them are disclosures, that all your information is checked to see that you are not a terrorist, laundering money, etc. If the attorney in fact signs, he is recognizing that this has been done on this file.

There may be more than one form out there so I may not be responding to what you have. Can you be more specific?

Last one I had with an AIF signing all docs, I put a post-it note on the Patriot Form that said
POA signing. Obviously you can't put down the I.D. information for someone that isn't there.
How do you get two or even one form of I.D. from the person that isn't there?

You list the info on the person signing, the AIF and that person signs own name. Adding a posit is not a bad idea.
