Can someone send me a version of their Notarial Evidence Form that I can use to type the info in and then print it out? I messed mine up and can't get it back where it was!

Charlotte, did not see it. Did you send it to [e-mail address]??? It reads as Q.com. As in Qwest not a 'G' as some people mistake it for.
