I wouldn't touch these forms with a ten foot pole

Their "jurat form" is a journal... they have a general "notary commission application". Yeah, the states will just love that. And it has to be "sworn to" and signed by an attorney. While you're on that page mosy on over to the "law office" forms - including a "divorce decree", "child custody form" and more!

I feel bad for the poor fool who thinks they can use any of those forms.
