Just like with dates, my policy is for no initials unless specifically asked for. I'm not a mind reader and unless title specifically tells me that they want something, I'm not going to presume that they do. While I do believe in the theory that "too much" is better than "too little", I have seen where a document was kicked back because a borrower mistakenly entered a date where it wasn't asked for. I have that same policy for initials. If there's not a place for it on the form and it isn't specifically asked for in the instructions, then no initials.
