My policy is to triple check docs before sending back.
Once, at the table.
Once more before sending.
One more time...before sending.
No focus-related mistakes. (or, unfocused mistakes, as the case may be).
No matter how hard we try to keep distractions at a minimum, distractions never fail to appear.
