the part that I watched of your video link was discussing face to face interviews. the addendum itself says you are to complete when face to face. to me that implies you do not complete if not face to face. since it isn't specifically addressed in the addendum itself, I can imagine some over zealous UW or staff attny directing LO's to complete. Maybe even Ginger Bell said to but I didn't listen to the whole video.
be that as it may, longevity does not equate to expertise as I learned in a state SOS seminar last year. 30 year presenter was completely incorrect on ID and requiring signer to sign exactly as ID shows name despite way written on a document. I discussed with the SOS chief who discussed with state attorneys and the presenter has been corrected.
