As for whether there are any states that put capacity in the certificate, several states have passed RULONA (although it hasn't gone into effect in all the states that passed it, yet). This law allows any certificate wording as long as all the required elements of the law are mentioned, but there are several short form certificates authorized. There are two different certificates for acknowledgements, one for an individual acting for him/herself, and one for an individual acting in a representative capacity; in the latter case, the notary writes in the capacity that the representative is claiming.

The other type of act discussed in this thread is a verification on oath or affirmation (that is, a person swears or affirms a written statement is true). There is only one short form certificate for this notarial act, and no place to write that the person was acting in a representative capacity. The RULONA has no mention of taking an oath or affirmation in a representative capacity; RULONA does not explicitly allow or forbid it.
