Just to be clear, the form I have the CWs sign is only to emphasize that they should all take the process seriously. I don't provide a copy to anyone, I just keep it for my own use and protection. I don't see it as my responsibility to not get the witness in trouble, rather to make sure they don't take on that responsibility lightly. After all, they ARE required to take an oath/affirmation confirming that the 5 listed prerequisites for a CW are true.
NotaryDi/CA makes a good point about title being concerned - and she would know! As for the lender's concern, there may be an issue with completing a Patriot Act form. (I believe that part is on them.)
