What...is this POA from the office supply store where one fills in the blanks? If so, the attorney can have his admin assistant go buy a new one, $10 - $20, the assistant can fill it out - info from old POA to new POA since nothing's changed, drive to their client, and get the new one notarized. I've notarized docs of various types drafted by attorneys and I've never seen where info needs "filling out". It's all pre-typed. The only thing the client does is initial and signs, where applicable. While CA notaries have a duty to serve the public, we do not have a duty to be fools. I wouldn't touch it either as I see red flags with this and wouldn't put myself in any precarious circumstances.
