Agree, Lisa. I had an ack the other day that read:

<<Acknowledged before me on such and such date.>> Period. That was it. No mention of the signer's name (which CO law requires.) So I hand wrote: "by Mr. Borrower." Also, had to write in a venue. If I had "my own" certificate, I could have/probably should have used that.

However, as to adding the signer's name on a jurat when CO specifically excludes it ... but doesn't necessarily forbid it ... I'm not so sure it's up to me to decide it should be included ....

Totally agree! It's obvious they have no idea what they're asking for half the time, not to mention the difference between an ack and jurat.
