I did this one time many years ago, but can't remember the reason. It was all in my journal so I wrote out a statement, signed it and faxed it to the SS. I kept the statement and it's in my journal with the line item of the notarization. Just saw it about a month ago when I was searching for another notarization someone wanted verification for.

I can't imagine not keeping a journal. I get a call about once a year from someone requesting info on a line item. It has saved my butt in a few instances.
