I include the signer's address - to make a statement to *them* that the notarization is serious business. It's also a good cross-reference in case the ID's a good fake, which may have the crooks photo and address, but the victim's DL #. In my journal, I put stickies over the prior signer's info for privacy sake so the next signer only sees their own info and no one else's. I request a thumbprint for every notarization and when I get docs notarized, I gladly give my own thumbprint, regardless of the type of doc.
