I would print extra copies and have them initial one and not the other and have borrower add note on separate sheet explaining that they are not exercising their right to cancel at this time, however, due to difficulty in interpreting lender intent with initial lines in the box, they are initialing in case required at loan consummation. please discard document not needed....or something to that effect.
whatever you do, don't let them sign in the box 
I would ONLY do this after trying to reach any and all persons involved and I would include explanation with my closing confirmation to hiring party.
