Some people will initial and date but just an initial should do it. I think an initial is a good idea.
We have all done this. Always shocking when it happens. I even put a small narrow green post a note, taped to the side so I could see it, so I know to stamp only if I can see the green.
Green = Go. Even with this I have stamped upside down. More than once.
Thankfully I make my own certificates so there is room to place another stamp for these occasions.
If there is not room to re-stamp then attach a loose certificate.
It is important to not act like you made a huge error. Just be casual otherwise the signer gets uneasy and worrisome. No need for that.
