I don't think I would have have used those certificates, either, and what you copied wouldn't have indicated to me that it needed notarizing, but more info is needed to say for absolute certain. Was this perhaps a commercial or construction loan? Was there more than one signature per person? Was there any pre-filled information on either of those certs? A bar code? Connecting page number?

If the answer is no to all of that, the next thing I'd be wondering is if the person who's asking me for those certificates to be completed really knows what they're talking about. Is this an escrow officer or their assistant? Someone at a signing services? How well do they know what they're doing or are they just making an assumption and trying to cover their rear ends? I don't think I'd want to go back on my own dime without a very good explanation - and/or perhaps asking for verification from someone else. Some of my favorite words in these types of situations: "Are you SURE?" Can't count how many times I've pushed back (as politely as I can) only to have them end up deciding to ask someone (although they don't always admit it) and then suddenly it's not so necessary...

IF you decide to provide an additional certificate, please carefully cross-reference the certificate with the document, even if it means hand writing in verbiage (below your signature and stamp) that states the name of the document, date, number of pages, etc. And under the signature notarized, I'd write "see attached certificate" so it's clear that it was a notarized signature - and so that certificate couldn't be separated and used on some other document.

Also, there's no way I'd give them more certificates than there are signatures on that document (if there are more than one), and each sig would have a cross-reference statement. I have seen commercial loans where they want a separate certificate for each signature, but those usually have the notary certificates bar coded and page-numbered along with the rest of the document, so there's no way they could be used inappropriately. If nothing like that is provided, we shouldn't be afraid to write it in, although with loan documents, that is seldom necessary. [I can think of at least one title co, though, where I do that consistently with their docs.]
