Question about CA DMV Form reg227
This is a 2-sided doc which can be used for a lot of tasks. The top of the form for Duplicate Title says to do Sections 1-3 (which do not say notarization is required) Section 5, however, needs to be notarized.

My client is applying for a Duplicate Title
I assume the person I'm helping should cross through un-used items on the first page. Should she do that for everything on Page 2 as well? Even if it for a transfer of ownership, and none of the notary text that also appears on page 1?
I'm going to try to get a live person at CA DMV as well, but wanted to know if anyone here has experience with that form.

The live techs at the DMV won't be able to help you much. CA DMV forms are a royal PITA. They often serve multiple purposes and many sections are intended to be left blank if they don't apply or are intended to be filled in later by other individuals or organizations as the paperwork passes through many hands.
As for notarizing... you are only notarizing the signature in Section 5. Everything else if filled out according to the situation of the individual and it is NOT your call as to what or how they do that. If they don't know, they need to talk to the DMV, not you. In sections 1-4... items will likely be left blank because they do not apply to the situation. It is up to the person to cross them out if they wish... but again, NOT your call. You can only tell them that you cannot notarize an incomplete document and if they need assistance filing it out, they need to talk to the DMV.
In this case, though, the person has to fill out section 1-3 and signing section 4. It's not that difficult. If something doesn't apply, they should probably just write 'N/A' -- but it's not my job to tell them that. It's kind of a delicate balance... we cannot tell them how to fill something out or what to write, we can really only refer them to the instructions or have them call the DMV and ask how to handle blank lines.
The second page is irrelevant. You only care about the information PREceding the notarial certificate. It can be left blank. But, FYI, per the Notary Workbook (http://www.sos.ca.gov/business/notary/forms/notary-education-sample-workbook-2013.pdf ) on page 26 it says, "...in determining whether the document is complete, the notary public should ignore information that is intended to be added later such as information to be placed on the document by a county clerk or recorder or for additional signers whose signatures are not being notarized by the notary public."
