I'm sure there are many, many places like that all over the country. They have no cases at all - until they do. All it takes is one person who has a visitor who doesn't realize they have it, or one person who had a business or shopping trip, or whatever, who happened to come into enough contact with someone who was recently exposed themselves... and they bring it back with them.
Contagions don't hit everywhere at once. They take some time to travel and the more remote an area, the longer it can take to get there, naturally. But it's probable that in our very mobile society, it eventually will. If a whole population leaves themselves vulnerable, it could spread very rapidly once it arrives.
I'm not trying to generate fear, but recommending being proactive. If simple precautions are taken before we know it's essential (by which time it will already have had opportunity to get well established in a community), we can prevent lots of problems. IMO, the countries that have had the best results with controlling the spread of COVID-19 used that type of approach. Just sayin'...
