The notary could overcome the problem of which notarial act is needed by waiting for the voter to do the research, and then the voter would tell the notary which act to perform; if it wasn't the right one, it isn't the notary's problem.

The notary would then have to find some justification for making the other statements that are on the envelope that are not notary duties; I don't know how I would overcome that problem.

If the notary can get past the above obstacles, the notary could write the certificate on the side of the envelope opposite from the side with the voter's signature, if there is room. The stamp could go on the side with the notary's signature, the side with the voter's signature, or both, since it's all the same piece of paper.
