It requires the veteran to complete a series of questions, which include the date he/she entered the military, date separated, and service number. This is for active service and also for reserve service. Then it also has a place to complete the info for any previous VA loans - date of loan and propery address for each propert, and attach a separate piece of paper if necessary to list all the loans and properties.
