That varies from one stamp to another. I've been wondering the same thing about the Stingray stamps I have with the required CA jurat verbiage. The top (blue?) piece comes off and it looks like there's a well where ink can be added, but I've never gotten around to asking. I have two of those and, naturally, they don't get used nearly as much as my notary stamps, so it's not been an issue for me yet.
For my notary stamp, my last two commissions I've bought the Vision Clarity stamps from here (love 'em) which do have an ink pad that can be removed, as LD described. I buy two, so I have one I keep with my notary case and another to swap out when it needs re-inking - 'cause it seems like that only happens around EOM, or when I'm too busy to want to deal with it...
