requires the venue (state & city where signing took place), notarial language (acknowedged before me or sworn to before me), date, your signature & stamp. If the block requires your notary commission date & #, be sure to complete that also.
As far as the borrowers initialling the page, some lenders require initials & some do not. If they are required, yes, they would initial that page too.
Some inexperienced notaries put their stamp wherever it says "seal" in the documents. Don't do that. That isn't meant for us. You only notarize within the notarial certificate. I understand CA requires specific language too. Most of us don't do loose certificates, but many in CA do.
