1. HIPAA did not need to be notarized (this was a NY/PA law firm)
2. No POA
3. Intake Q-form was completed by firm. They had his name & add wrong, so he corrected. Widowed, not divorced as they had.
4. Legal Retainer (same as was mentioned)
5. Social Media?? No such animal in my packet.


Summation: Fairly easy. 7 pages to print (I did leave him an extra copy of the Law Firm Agreement & his questionnaire so he had their contact). Scanned back w/ID. Very pleasant gentleman, despite all he's gone through. Only negative: a veteran living as he is. Plus he admitted to being a smoker.
Not sure if he'll be around for whatever the outcome. My heart did go out to him and I asked if there was anything he needed.
