Did you take an online or live class. II would also advise you to use the word "and" between names, such as Jack Smith and Jill Smith. I would not write Jack Smith & Jill Smith. If there are more than two names, then use commas, then "and" before the last name. Dates should be written out as in Jan. 13, 2015 or January 13, 2015. Bad idea to write 1/13/15.....easy to alter all numbers.
