I get your inference. OP does not make clear if the doc in question is completely blank (devoid of any and all info) or simply has borrower's name on top line and the rest is left blank in case there are any AKAs to list. As someone else has posted ... sometimes lender will send a form with info and another form with no info at all (maybe just in case the first page wasn't complete for all AKAs or in case of maybe a non-borrowing partner whose credit report was not pulled or maybe just an extra for whatever reason, IDK). In this latter case, if that second page is not needed, I would simply draw a line across it, write N/A and have borrower initial.
