It's documentation for a private (not class-action) lawsuit against a drug manufacturer for undisclosed side effects.
Which, I believe, means you'll be at the table for quite some TIME while they fill out forms requiring a lot of info. At least that's been my experience with similar...DrS/Hospital names, addresses, dateS of treatmentS, insurance info..and on & on.
