"Signing Agent Declaration"

Has anyone signed this document? I am having issues with the "under penalty of perjury of the laws of the State of ___________ that the following is true and accurate" part. It then goes on to say " I have completed a training course that is the basis for me being able to provide sales presentations for ******** Law Group." I was OK with this until I got to this document.
