The situation where I'd attach a jurat would be this imaginary document:

This record was acknowledged before be on October 15, 2018 by John Doe.

So the document came with an acknowledgement certificate, but it still doesn't have anything signed by me saying that I administered an oath. John Doe wrote and signed that I administered an oath, but I didn't write anything down about that. So I have to attach a certificate saying I administered an oath.
