Would you strike thru the Title of . . . Or Authorized Agent? 1st block in the Certification is for Signature of Employer/AA

(Also, more I think about it using Staffing co. name seems best; all this 3rd party stuff is where it gets confusing for me)

I wonder if staffing/hiring agency thought they were being a great help to their Employer/client by offering to take care of this for them? Frankly, then THEY should have someone as Agent at the staffing firm - imo.
