a change due to marriage. When a court changes your name, the birth certificTe is supposed to be amended to state the new name. This doesn’t apply to changes such as through a marriage or divorce.

The application to amend the birth certificate is just an affidavit. It’s not a vital record, but even if it was, a Notary CAN notarize a birth record... such as a record that includes a paternity affidavit. What they can’t do is certify a copy of a birth record.
