If not, you're not; you need to clarify with hiring party exactly what they want you to do - and I suspect you'll go to facility and notarize a signature, possibly get a copy of the signed Affidavit to scan back to them, possibly take the medical records to drop at courier (which I would not, personally, do - I don't want anyone's medical records in my hands) - follow your notary handbook as to proper notarization procedure.
