Yeah, I think you may be 'splitting hairs' worrying about the method it was sent. The requirements just state what info is needed, not anything about the format used to provide it, other than that it's in writing. I've interpreted that broadly on the few occasions when I've been asked for that info, including via email.

If what you've received includes the necessary info ("the name of the parties, the type of document, and the month and year in which the document was notarized"), I'd be inclined to accept it and just send the line item (being careful to block out any adjacent data).
