When I was first faced with the boxed disclaimer requirement I called Harry Shoemaker and had him make me a disclaimer stamp.He made a good one and I've used it along with an acknowledgment stamp a thousand times. My jurat stamp has the boxed disclaimer incorporated in the stamp.
Here's a GNW trick from an old Pro.
Have you ever got presented with a US government document with a non-compliant jurat and then stapled a compliant one to the signed document? Well I know from experience that all of you do. By the luck of the draw, I occasionally I get a GNW call from someone who had their document rejected because of the loose attachment and this is how I fix it.
I use my jurat stamp and my notary stamp right on top the document wording being careful not to cover the signers signature. I then fill out the jurat and sign it. Sometimes it's almost unreadable but I have never had one returned.
