The way I read the original post, the notary's name in the jurat was indicating that it was the notary who was having an oath administered to him/her. If that's so, then it wouldn't be enough to have a separate ack and oath certificate, the notary would have to go before a 2nd notary and have the 2nd notary administer an oath to the 1st notary.

There are other discussion boards out there that let people attach photos (with suitable redactions). The technology in this board makes it hard to come to correct conclusions.
