Some places may actually call the voter if there are any problems, giving them the chance to fix the error. (I'm confident that's not done over the phone, but I don't know the process.) Even here in populous CA, there are often opportunities for fixes to be made. I heard about one example on the news yesterday.
Some idiot actual set on fire the contents of a ballot drop box. An election official said that they were able to identify which ballots were damaged [don't ask me how] and that those voters were being contacted. What I DO know is that each ballot has a bar code and is tracked through every step of the process. We have the option to sign up for notifications as our ballot passes through each stage. I received an email before my ballot was even put into the mail to me, which had me confused at first. I received another one once it was delivered.
It appears that the election process in many (hopefully most) places has gotten pretty sophisticated, although I don't doubt there's still room for improvement.
