If you register on the Do Not Call list, compliant telemarketers should remove (scrub) your phone number from their calling lists. Non-compliant telemarketers do not scrub their lists and make unsolicited calls in violation of the Telephone Communication Protection Act (TCPA).
Most people that receive spam phone calls hang up, or say no thanks, I'm not interested, stop calling me, or put me on your do not call list. Some people install spam blocking phone apps. Some will file an online complaint at http://www.donotcall.gov/
Sometimes, the Federal Trade Commission (FTC) will take action against Do Not Call List TCPA violators. In this case, the FTC is not a party. The Do Not Call list did not stop the unsolicited phone call.
