I do about 150 of these a year. I have learned, with advice, to be sure there is witnesses on hand. I will inform who ever is calling that there needs to be a medical type person familiar with the patient and the next of kin, E contact, or spouse. Most all of the patients are very aware of what is being signed and why... BIGGEST issue is the hospital visits,,, seems like 10 members of the family are there, talking over each other, and hate each other.
