I leave that form alone, because it just informs them that this is how their name has been pulled from a variety of sources, and if they do not like it, they need to investigate it later why it came like that. This cannot be marked up.
The one where it is on one side and they sign on the other is where they can sign "never known as" instead of acknowledging it.
I used to let them mark up until I was told not to, so now it's just an informative page basically. Just what I do with it. |
