What I've been doing is ...

1. initials on first page (if applicable - joint app), about an inch below borrower's name under "Type of Credit";
2. signature on dedicated signature page*;
3. and, sometimes when more than one borrower, there's additional signature page that requires signature again*.

*IME and regardless if married couple with/without same last name, sometimes each borrower has his/her own application and signature page; sometimes, they're both on one app and one signature page. Not very "UNIFORM", is it? Wink
