Old Republic Title

Has anyone worked with Old Republic Title company. I recently submitted an application with them. According to the email they pay $95 per signing. However I was requested to complete a sheet giving my signing fees. Can someone guide me on what to do cuz I don't want to over or out price myself. I by the way I mean new notary in New York City, Kings County
