These are very important documents. The doc's are State Specific. My local Care Center sets the
appointment with me, then we gather for the signatures. The Administrator provides the doc's
both AHCD and Financial POA. Plus the two witnesses, who by now are trained! She also provides copies
to all parties, and I provide instructions for the A.I.F. The Nursing center makes sure all residents are
covered by these doc's. Have been doing these for some 25 years at the local center.
Many years ago, when my father started having health issues he confided in me he was worried that if
he ended up in hospital he did not wish to be hooked up to tubes! I told him we could fix that. That was fifty years ago. After papers signed, he finally slept at night.
