Derrick pointed you in the right direction, Melanie, there is an example in your notary handbook (copy at http://www.sos.state.ms.us/busserv/notaries/NotaryGuide2004.pdf),

The very last page is the Notary Application containing a jurat at the bottom, because you have to swear or affirm that the information you provide on the application is true.
