I've seen a W-9 & even a 4506-T. I had one borrower adamantly refuse to sign the 4506-T. He obviously had tax issues that he didn't want to come up. He wasn't on the loan, but if he's on title a child-support or tax lien could be an issue for the property.
Again, that's my opinion & I don't express it except for here on the forum.
