CA doesn't have signature by Proxy, just Signature by Mark, but the procedure required may be useful for that situation, as well. Two witnesses are required, both of whom have to sign the document. On the signature line, one of them (if memory serves) signs the principal's name for them along with their own. For example, they would sign "Robert Principal by Mary Witness". Then Mary and 2nd witness both sign and print their name on the document as witnesses. (Going from memory here, so please take that into consideration... Wink) You could also consider having the witnesses include their addresses, as with other types of witnessed docs.
