I have enough to deal with with my own state's NP laws, BUT

I have seen (some years back) an alleged CA compliant form that
contained a section following the standard ack form showing
"Capacity claimed by signer" with boxes to check for corporate officer, trustee, attorney in
fact, etc,
Would that still be acceptable under CA law?
