FYI: In the footer of the form it specifies the following:

As such, doesn't appear to be an updated form. May very well be as you speculate, Bear, that it's lender specific as this is my 'maiden voyage' with lender. Wink Quite unusual to see signature blocks on each and every page of the loan application/Form 1003. TYVM
