Is this template about the size of a credit card? I had a signing about 5 years ago with a blind son and his mother. She had cut out the center of a credit card and used that as a guide for her son. It looked very similar to the picture of the template you posted. I'm much more comfortable if someone else, besides me, guides the signer to where to sign.
