Normally, I don't put a description of the doc (beyond "foregoing", "within", etc.) unless I'm attaching a loose cert, then I put the description at the bottom of the page, outside the wording of the acknowledgement.

That said, I have no objection to including a longer description of the document within the certificate.

LynnNC's excerpt doesn't have a place for the page number. I would count the acknowledgement certificate as one of the pages of the document if it has the signer's signature on the same page, or if it has one of those bar codes at the bottom like the other pages. If I'm attaching a loose cert, I don't count the loose cert as one of the document pages.

I haven't had a situation where I thought people would be confused, but if there was room for confusion, I'd write something like

"...acknowledged the due execution of the foregoing instrument, a Deed of Trust, comprising 14 pages exclusive of this page."
