Wow, maybe he had a disease or something at that age. That's a lot of pressure to put on a kid to figure out how they want to live or die if sick or incapacitated. A lot would factor into consideration about his life with his wife and children too.
It's good to pre-plan, but that is young at 18 to decide as they have no clue yet about life. jmo
