It doesn't work like that here or in WI. If you are sick and incapacitated the next of kin are notified, which would-be parents if not married and no children. If parents not around, it goes to the siblings. Maybe he doesn't want his family to decide? The state only takes charge of a John Doe and no family can be found.
Maybe he hates his family or they are just off the chain cray cray and do not want or allow transfusions and alike, then I would def. do that...Not saying it's a bad thing, just unusual when a teenager is doing an AHCD.
