FWIW, I do the same as you. I had a kind of similar situation this week with a bank document that a second signer had recently already had notarized by someone at the bank. This notary used the provided certificate, which might have been OK about 3 major changes ago... (It still had "personally known or proved to me..." and no box disclaimer.) I attached a compliant certificate for the signer I notarized and left them with an extra business card, since the form was to be sent back to the bank. [The other notary's commission expires next month, so I hope she doesn't renew!]

I've yet to have anyone contact me with questions, but my hope is always that the other notary will do some research and figure out what they're doing wrong.
